ADMISSIONS

Admission to the school is by application only. There are three forms which must be completed in full and submitted to the school in order to consider your child for admission.

The forms are:

The Enrollment Agreement Form

The Emergency Contact/Parental Consent Form

The Health Information Form

The Health Information Form is completed by the child's pediatrician. There may be a small charge to you from your doctor to complete this form. All three forms should then be mailed or brought to the school for evaluation.
